This page contains a link to an evidence table designed to be used in conjunction with Dr. Lake's book An Integrative Paradigm for Mental Healthcare: Ideas and Methods Shaping the Future. The evidence table is designed to help clinicians organize information on non-pharmacologic treatment approaches when developing an integrative treatment plan.

To download the evidence table please right click the below link and open with Adobe Reader. After filling in the evidence table you can save it on your computer for future use. I suggest saving the blank evidence table as a template so that you can use it when working with different patients.
