Treatment plan

This page contains a link to a template intended for use by clinicians in conjunction with my book. The template allows you to organize information when developing an individualized treatment plan addressing the symptoms and preferences of each patient. I suggest downloading the blank template and saving it in Adobe Acrobat. You can then use the blank template to create a treatment plan that is customized to each patient's unique needs.
